Form “C”

Form C is a short form of The Bombay Nursing Home Certificate. This Certificate Called Hospital Registration Certificate. This Certificate will be Issued by local Supervising Authority,

Form “B” Certificates

Form B required to be submitted Twice a year or Six Monthly by License Fire Agency under the provisions of Section 3 of Maharashtra Fire Prevention Act.

Fire Safety Audit

Multi-storeyed hospitals have the following differences in the evacuation of its inmates. A majority of the patients are ambulant or with life-supporting equipment and need external assistance for safe evacuation. Hence special attention needs to be paid for the safe evacuation which may be partial i.e. moving the patients of the floor where the fire has originated and the patients in the floors above and below the floor of fire or total evacuation i.e. safe evacuation of all inmates of all the floors to a safe place outside the building. In a big Multi-Storey Hospital, a majority of the visitors and attending relatives of the in-house patients are not familiar with the surroundings and hence incapable of offending their way to a safe place during an emergency. Unlike other occupancies, special hazards are associated with the equipment, materials used for treatment, various hazardous gases like ethylene, chloroform, ether, cyclopropane, and ethyl alcohol, liquid oxygen, used for operation theatres. There are too many people in the hospital premises who can commit an act of negligence which may result in fire and too many equipment which are capable of providing a source of ignition i.e. electrical heaters, laundry, sterilization plant, presence of a large quantity of Liquid oxygen, LPG supplies always turn small fires into an uncontrollable inferno. Therefore, initiation of fires in hospitals in the absence of fire prevention management is highly possible. Basic design and construction faults enable fire to spread and lack of proper escape routes and evacuation procedures place the life of people in danger.

Fire Safety Renewal

The hospital authority shall renew their fire safety licence every 6 month. they should also refill their fire extinguisher per year and also submit the Fire extinguisher refilling certificate with the fire licence copy to the fire department.

MTP (Medical Termination of Pregnancy)

MTP, which stands for Medical Termination of Pregnancy, is a procedure of terminating pregnancy using medicines. In the early stages of pregnancy, it can be terminated with the help of medicine, otherwise, the surgical process is needed. If the length of pregnancy is not exceeded 20 weeks then, medical termination of pregnancy is legal in India. Legally, only the consent of a major pregnant woman is needed in terminating the pregnancy and no other person’s consent needs to be obtained.

Tubal ligation (commonly known as having one’s “tubes tied“) is a surgical procedure for female sterilization in which the fallopian tubes are permanently blocked, clipped or removed. This prevents the fertilization of eggs by sperm and thus the implantation of a fertilized egg. Tubal ligation is considered a permanent method of sterilization and birth control.

The Hospital Authority Shall having a Valid Bombay Nursing Home Certificate from Local Supervising Authority

PCPNDT Registration

Pre-Conception and Pre-Natal Diagnostic Techniques (PCPNDT).

Register all the places wherever USG machine/any imaging machine like Color Doppler/2D/3D/4D machine is used, including medical/ surgical Nursing Home/ICCU wherever USG machine is used. There should be only one machine for one center, and the machine should not be shifted from one center to other center. The company name & make of machine should be noted in PCPNDT registration certificate. All radiologist /sinologist / Doctors using machines, their names to be entered in the PCPNDT certificate or on the separate sheet to be displayed along with PCPNDT registration certificate.

MTP & PCPNDT Report Submission

The Hospital Authority Shall be Submit their monthly Report’s. To the local Supervising Authority before 5th Day of Every Month.

Birth Form Submission

The Hospital Authority Should Submit the Birth and Death Record forms to the local Supervising Authority within 21 Day’s.
